Non-Medicaid Care Management

ABOUT THE PROGRAM

Non-Medicaid Care Management brings equal access to residents by providing innovative care coordination to children and youth who are experiencing emotional and behavioral difficulties in the home, school, and community settings. Non-Medicaid Care Management staff provide a continuum of mental health care to children, youth, and families by identifying strengths, needs, and concerns. Families, community providers, and school staff all work together to develop an action plan for success.

The Non-Medicaid Care Management program serves all school districts in Broome County, New York.

GOALS

Children, youth, and families welcome the opportunity to take ownership in their circumstances and work toward the following goals to enrich their quality of life:

  • ·         Attend mental health appointments
  • ·         Reduce the need for crisis intervention services
  • ·         Improve academic performance and increase school attendance
  • ·         Access appropriate community resources to improve circumstances
