ABOUT THE PROGRAM

The Adult Non-Medicaid Care Program provides care management services to adults living with severe and persistent mental illness and supports them in living a safe, healthy, and enriched life.

GOALS

Support, education, and advocacy empower participants to take ownership in their circumstances allowing them to achieve the following goals:

Regularly attend mental health appointments with psychiatrists, therapists, and case managers to proactively address mental health wellness and recovery.

Increase self-awareness of signs/symptoms of mental illness in order to decrease the need for crisis services and/or psychiatric hospitalization.

Manage daily living needs such as food, clothing, transportation, and mental and physical wellness.
